HealthRADAR offers many features that help you stay connected with your care team and feel motivated.
Simplified sign-up and collect health kit
Once you sign up for chronic care management at your clinic, you will receive an enrollment link and health devices to monitor your health at home.
Care coordination and in-built coaching
Your doctor will discuss and establish an individualized at-home care plan for you. The HealthRADAR app will automatically send you reminders and initiate first-time data entry at a regular frequency prescribed by your doctor. As an added benefit, you will also receive motivational quotes and health tips to keep you informed and motivated.
Monitoring
All your recorded health data is securely shared with your doctors and monitoring nurses. At any time, if your reported conditions show signs of worsening, HealthRADAR alerts your doctors and nurses for appropriate action and advice, which they can share over a phone call or via secured chat.
Secure Chat
You can stay connected using secured messaging via the HealthRADAR app. Your messages are always safe and never used outside of sharing with your nurses and doctors.
Weekly Progress Reports
You and your doctor will receive a weekly report showing trends and problem areas, which you can discuss with your doctor. Your doctor may suggest changes to your at-home care plan.
